Salary: $65,000 - $90,000 per year A bit about us: We are a
non-profit organization working with California residents that are
in need of Medical, Dental, Behavioral, Nutrition, and Optometry
services. As part of this interdisciplinary team, our providers and
clinicians cross collaborate to provide the best patient care
possible. In this particular role, you will be providing Nutrition
services to our PACE (Providing All Inclusive Care to the Elderly)
patients in Visalia, CA. Some of the daily duties include:
Providing Initial Nutrition Assessments Developing a Care Plan with
the Patients Implementing/Managing the Care Plan Working with the

/> Job Details Job Details: We are seeking an experienced,
dedicated and dynamic Dietitian/Nutritionist to join our healthcare
team in a PACE setting. The successful candidate will play a
critical part in the wellbeing of our patients, providing
individualized nutritional guidance and care. This role is perfect
for a professional who is passionate about health and wellness,
enjoys working with diverse patient populations, and is eager to
make a positive impact in the lives of patients through nutritional
education and counseling. As with all PACE programs, the patient
population is working with older adults who are enrolled in an
interdisciplinary healthcare team. Responsibilities: 1. Assess
nutritional needs, diet restrictions, and current health plans of
patients to develop and implement dietary-care plans and provide
nutritional counseling. 2. Consult with physicians and healthcare
personnel to determine nutritional needs and diet restrictions of
patients. 3. Advise patients and their families on nutritional
principles, dietary plans, diet modifications, and food selection
and preparation. 4. Monitor food service operations to ensure
conformance to nutritional, safety, sanitation and quality
standards. 5. Coordinate diet counseling services. 6. Analyze
nutritional content of food to ensure meals meet specific dietary
requirements. 7. Document patient care by charting in patient and
department records. 8. Keep up-to-date with the latest nutritional
science research. 9. Educate health care team members, including
physicians and nurses, on the importance of nutrition, diet and the
role of the dietitian. Qualifications: 1. Bachelor’s degree in
Nutrition, Dietetics or related field. 2. Must be a Registered
Dietitian (RD) or Registered Nutritionist. 3. Minimum of 1 year of
experience as a Clinical Dietitian or Clinical Nutritionist. 4.
Proven experience in the healthcare industry. 5. Excellent
communication and interpersonal skills with the ability to
translate complex nutritional concepts into simple and practical
advice. 6. Strong understanding of the clinical nutritional
requirements for various medical conditions. 7. Proficiency in
dietetic software and patient record systems. 8. Ability to work
independently and as part of a multidisciplinary team. 9.
Demonstrated ability to manage a diverse caseload effectively and
efficiently. 10. Commitment to continuous professional development
and willingness to stay updated on the latest advancements in the
