Occupational Therapist (OT)*The clinician hired for this position will be treating patients in the Fresno and surrounding areas.POSITION SUMMARY:As a Home Health Occupational Therapist, you will evaluate and treat patients with injuries, illnesses, or disabilities in their homes. You will help patients recover, improve, and maintain skills needed for daily living.RESPONSIBILITIES:Evaluate patient functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.Evaluate the home environment for hazards or barriers to more independent livingIdentify equipment needsProvide supervision to the Occupational Therapy AssistantsConduct an initial evaluation/assessment and report findings to the Health Care Provider and RN and develop the plan of care promptly for patientsDocument initial evaluation and treatment plan and re-evaluations of patient's progress at least every thirty (30) days and update plan of care as necessary.Complete comprehensive assessments and OASIS data, as indicated on a timely basis.Observe, record, and report to the Health Care Provider the patient's treatment, response, and progress and promptly report significant changesObtain orders to revise the plan of careInstruct patient/CG in the use of alternate methods and devices for upgrading independence in activities of daily livingOther duties as assignedQUALIFICATIONS:Bachelor's Degree and active Occupational Therapist license in the state of practice.Work experience of at least one (1) year under a qualified Occupational Therapist, home health experience preferable.Must have a vehicle, valid driver's license and automobile insuranceAbility to make home visits and provide occupational therapy evaluations and treatments in the home environment complete documentation effectively and submit in a timely mannerHome Health and Home Care Home Base documentation experienceBENEFITS (available to full-time employees):Excellent pay, including 23 days of PTO per yearHealth, dental, vision, 401k, life insurance, short-term and long-term disabilityContinuing education & tuition assistanceGenerous referral bonus programFlexible schedulingCompany Vehicle Program with a paid gas card for eligible full-time employeesABOUT US:Our unique focus is on patient-centered care and evidence-based medicine.
