The Home Health Physical Therapist is responsible for delivering personalized rehabilitation services to patients in their homes, focusing on improving long-term health and wellness. This role involves conducting comprehensive assessments, developing individualized treatment plans tailored to each patients goals.RESPONSIBILITIES:Conduct an initial evaluation/assessment and report findings to the physician / provider and Clinical Manager and develop the plan of care promptly for patientsPerform physical therapy interventions, procedures, measurements, evaluations and diagnostic tests on patients in the homeDocument initial evaluation and treatment plan and re-evaluations of patient's progress every thirty (30) days and updates plan of care as necessaryComplete comprehensive assessments and OASIS data, as indicated on a timely basisAssist with orientation, supervision and consultation services to Physical Therapy AssistantsObserve, record and report to Health Care Provider the patient's treatment, response and progress and promptly report significant changes and obtain orders to revise plan of careCommunicate with physician / provider and Health Care Provider and other members of the health care team for the delivery and coordination of quality physical therapy servicesProvides education and support to patients and their familiesOther duties as assignedWHO WE'RE LOOKING FOR:You are passionate about restoring function, relieving pain, and preventing disability by engaging with patients in the most meaningful setting - the home.
