Job Description:

Overview:Under the direction of the Clinical Services Manager the Contact Tracer is responsible for reaching out to people (contacts) who have been exposed to or tested positive to the Novel Corona Virus (COVID-19) and providing health education and guidance to interrupt ongoing disease transmission. Dialogue with contacts will be guided by standard protocols to obtain any symptom history and other relevant health information, provide instructions for self-quarantine, and make appropriate referrals to testing, clinical services, and other essential support services. This position will require prioritization, prompt action, and attention to detail in documentation and data management as multiple investigations will be conducted simultaneously. Contract tracers are required to follow all designated scripts and comply with policies and procedures provided by the United Health Centers and the Fresno County Department of Health regarding confidentiality and data security for the handling of sensitive client information and protected health information. Approaches work in a consistent and timely manner and as directed by the Clinical Services Manager.Responsibilities:Summary of Responsibilities:

  • Initiate prompt contact tracing of any individuals UHC performed a COVID-19 test on and the result is positive or if UHC is contacted by County requesting UHC to conduct contact tracing of an individual that may have been exposed to or tested positive for COVID-19.
  • Provide education and a copy of the isolation orders found on the FCDPH's website to individuals who have tested positive and quarantine orders to household contacts and their close non-household contacts.
  • Conduct contact tracing by phone, in-person or via video appointment, and Contact Tracing work processes established by Fresno County. (Processes are subject to change depending on new information, technology and changing needs and may include use of both electronic and paperwork processes.)
  • Make a minimum of three attempts of varying communication methods to contact all individuals identified by County for contact tracing within 72 hours of being provided the individual's name and contact information.
  • For a confirmed positive result, complete the California Department of Public Health's required, established protocols for infectious disease reporting, including completing and submitting the California Department of Public Health - Confidential Morbidity Report (CDPH CMR).
  • Complete reporting indicating number of contacts made, number of attempts for those unable to contact. Reporting may be completed via completing County templated reports and/or data entry into County/State designated data systems, i.e. RED CAP, CalREDIE, CalConnect.
  • Provide approved information and guidance on quarantine procedures and what to do if symptoms develop.
  • Assess and facilitate referrals for testing and medical evaluation for individuals who become symptomatic.PERFORMANCE AREA 1: Operational Support Responsibilities
    • Collaborate and coordinate with a team of public health professionals (e.g., case investigators, surveillance coordinators, surveillance triage staff, care resource coordinators, epidemiologists) to efficiently complete contact notification and monitoring assignments.
    • Inform the team and supervisor when attempts to communicate with a contact are unsuccessful. Elevate complex situations to supervisor for further guidance.
    • Effectively interfaces with other departments within the clinic and corporation to ensure operational effectiveness for the area.
    • Effectively communicates and works closely with the Contact Tracing team, Operations team and health department representatives
    • Comply with compliance issues and regulatory expectations.Qualifications:EDUCATION
      • High School graduate or GED.
      • Completed training program in Medical Office Assisting from Accredited College is preferred.LICENSE/CERTIFICATION:
        • Possess a valid Driver's license.PRIOR EXPERIENCE:
          • Minimum 2 years of experience in a healthcare setting, preferably in an ambulatory care setting
          • Experience conducting telephone-based or in-person interview, data collection and data entry are preferred
          • Experience with health education, community outreach, linkage to care, social services, or other public health services.SKILLS
            • Bilingual Preferred (English/Spanish)
            • Strong verbal and written communication skills, including active listening and emotional intelligence
            • Able to quickly build and maintain rapport with patients and providers of differing backgrounds; team player
            • Ability to interact in a non-judgmental manner with a culturally diverse population and persons experiencing a wide range of social conditions
            • Ability to use discretion, maintain confidentiality and ethical conduct
            • Excellent oral and written communication skills
            • Customer-service oriented
            • Ability to adapt to changing environments and receive constructive feedback 3

            • Proficiency with MS Office 365 (i.e., Outlook, Word, Excel)
