You love what you do, now love where you do it. /n/nResponsibilities:/n Community Subacute & Transitional Care Center is a 106-bed facility providing inpatient care for patients with chronic and subacute conditions — such as coma, ventilator dependence, or complex illnesses — that require 24-hour medical supervision. With a focus on comfort, recovery and rehabilitation, CSTCC blends the effectiveness of acute treatment with the supportive environment of long-term care, helping patients progress toward improved function and quality of life. - Perform physical therapy examinations, evaluations, and individualized treatment planning to restore mobility and improve physical function. - Participate in interdisciplinary case conferences, discharge planning, and the coordination of care across the rehab team. - Implement a plan of care for patients and evaluate the effectiveness of it in an ongoing, systematic process. - Supervise physical therapist assistants, rehab therapy aides, and student clinical experiences to ensure quality care and professional development. - Guide therapy and intervention through evidence-based research As part of the Valley’s largest healthcare system, this role gives you the opportunity to apply advanced therapy skills in a subacute setting while helping patients achieve meaningful recovery goals. /n/nQualifications:/n Education & Experience - Bachelor's Degree in Physical Therapy required - 2 years of experience required Licenses and Certifications - PT - State of California Physical Therapist License required - BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required /n/nDisclaimers:/n • Pay ranges listed are an estimate and subject to change. • If any bonuses are noted, they are only applicable to external hires meeting criteria.
