to the Stroke Program Coordinator, the Stroke Program Nurse
Navigator/Physician Assistant provides expert nursing consultation
and practice oversight, functioning as a liaison between medical
staff, nursing staff, family members and support services.
Coordinates multidisciplinary care based on evidence based
practice. Responsible for providing stroke education, delivery and
oversight of patient care, including post discharge follow up.
Works in collaboration with Stroke Program key stakeholders to meet
stroke program certification requirements, including review of data
and performance improvement projects. Incorporates the patient
experience in all aspects of patient care. This position is a full
time exempt. /n/n/n /n/n/n /n /n Qualifications /n /n /n /n - CA
RN/NP/furnishing License or CA Physician Assistant License /n - BLS
/n - ACLS /n - NPI /n - NIHSS (within 3 months of hire/transfer) /n
- ASLS (within 3 months of hire/transfer) /n - ENLS (within 6
months of hire/transfer). /n - SCRN/CNRN/NVRN preferred /n /n The
stroke team is willing to consider any candidate with strong ICU,
neuro, or ED experience. /n /n LI-AR2 /n Tenet complies with
federal, state, and/or local laws regarding mandatory vaccination
