IDR Healthcare is seeking a local contract nurse RN Case Management for a local contract nursing job in Fresno, California. Job Description & Requirements - Specialty: Case Management - Discipline: RN - Duration: 26 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract This position is hybrid-four days out in the field and 1 day at home, focusing on conducting comprehensive annual wellness exams and focused screenings in a virtual environment. Clinicians will play an integral role in helping patients live healthier lives while providing high-quality care. Key Responsibilities - Counsel and educate patients and families about available benefits and programs to improve their health. - Document key items such as: - Chief complaints - Applicable diagnoses - Past medical, family, and social history - Review of systems, examinations, medications, allergies, assessments, and care plans. - Ensure all documentation is completed accurately and in a timely manner. - Maintain the quality of care standards as defined by the practice. - Assist patients with enrolling to access educational videos and other resources. - Participate in integrated care team meetings. - Conduct health assessments, implement care plans, and follow up to evaluate patient progress. - Apply knowledge of disease diagnosis and prevention to patient care. Additional Information - Equipment: A company laptop is provided for all roles. - Training: - Virtual training for 2 weeks, with 3 days of in-person training in Nashville, TN (travel is fully covered). - You will be paid during training. - Start Dates: - Time: Monday-Friday 8am-5pm - Support: A collaborating physician will be available for APPs. This role is ideal for clinicians passionate about virtual care, patient education, and making a difference in chronic care management.
