Pay Information $2,826 per week About The Position Job Title: Case Manager Profession: Registered Nurse Specialty: Care Coordination Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: 2 years in Case Management in an Acute Care Setting License: Valid State RN License required Certifications: Current certifications from AHA or American Red Cross required Must-Have: - Experience in Epic charting preferred - Background check must be clear of significant hits Description: Responsible for Care Coordination and Care Transitions Planning throughout the acute care patient experience. This position collaborates with the Physician, Utilization Manager, Medical Social Worker, and bedside RN to ensure timely progression and transition of patients to the appropriate level of care. The role aims to prevent unnecessary admissions or readmissions. The Care Management process encompasses communication and facilitates care across the continuum through effective resource coordination. Goals include achieving optimal health, access to care, and appropriate utilization of resources, balanced with the patients' self-determination. Collaboration involves patients, families, physicians, the interdisciplinary team, nursing management, quality and ancillary services, third-party payers, review agencies, claims and finance departments, Medical Directors, contracted providers, and community resources.
