* Up to $25K in bonus incentives (sign-on and relocation) *

and those we serve. You will join a committed team of over 100 Home
Health professionals with a fervor for doing good work in our
community by initiating, intervening, and evaluating nursing
services to meet patients where they are within a 60 mile radius in
the surrounding Fresno area. Community is expanding its footprint
and that means bringing on more staff to cover our Home Health
Department.

As a Home Health Registered Nurse, you will have the opportunity to
collaborate with a team of Home Health professionals with a passion
for caring for Valley patients in the safest place to them, their
home. Averaging 35-30 visits per week, you'll have the unique
opportunity to focus on the continuum of care for a wide acuity of
patients, including establishing a plan of care. This is the
