Visiting RN, LVN, CHHA, MSW and SCDetails:Urgent openings for Visiting RNs, LVNs, and CHHAs in FRESNO and surrounding areas ! Part-time/Full-time. Also looking for PTs, Spiritual Counselors and MSWs.We are a home health agency that need a team of disciplines to visit & care for patients at their place of residence.We also need another team of disciplines for our hospice agency.PREFERABLY SPEAK/UNDERSTAND SPANISHPosition: SPANISH SPEAKING LVN, PT, RN, SC, MSW, CHHA (Part Time Position)Duties:RN, duly licensed in State of California with experience in home health, hospice, facility or hospital. Willing to do home visits. RNs to assess patient's condition and determine home care or hospice needs, initiates/develops plan of care with MD and coordinates with inter-disciplinary team and ensure progress toward goals. To provide quality medical care, medication teaching and administration, health education, emotional support and guidance, safety protocols, and referrals to ancillary services.LVN, with California license, to assist in the nursing process by instructing patients and care givers in self care and providing direct patient care as needed. To visit patient once or several times a week as assigned.Certified CHHA, to assist with patient's activities of daily living and personal hygiene. Includes bathing, dressing, grooming, feeding, toileting, and patient's safe transfer/ambulation with/without the use of assistive devices. To help provide/maintain a safe environment for patient and do other tasks as CHHA job entails.Social Workers to help patients and their families navigate planning for home care or end-of-life care, know their needs and connect to resources available within and outside of home health and hospice settings to help them.Spiritual Counselors to assess spiritual needs of patients and families/caregivers and provide direct spiritual care and appropriate services to them.Physical Therapist to assess and develop personalized treatment plans, perform treatment to increase mobility and teach patients exercise techniques to improve their health.Qualifications:Current and unencumbered CA license for Registered Nurse and LVNs. CHHA must be CA certified.For Spiritual Counselor, have graduated from accredited divinity school, 1 year experience in spiritual counseling in health care setting.For MSW, a graduate of bachelor's program in social work with 1 year experience in health care.For PT, licensed to practice as physical therapist within the State of California with at least 1 year home health care experience.Willing and comfortable to visit patients in the privacy of their homes/residences.Understanding and commitment to home health/hospice philosophies and recognize the unique care needs of every patient to offer a compassionate, respectful, dignified and high level quality care.Must demonstrate strong organizational skills, leadership, and good verbal/written communication in English and preferably in Spanish.Valid BLS/CPR certification and completed Covid 19 Vaccination.Valid driver's license, auto insurance, reliable transportation, and willingness to drive to patient locations.JOIN our team of professionals and assist communities with exceptional quality care!Send your resume or inquiries today and we will call you ASAP!
