The Community Subacute and Transitional Care Center is a 106-bed skilled nursing facility (SNF) comprised of two unique and distinct units: Subacute and Transitional levels of care. This facility provides inpatient care for chronic, subacute conditions and other complex illnesses in addition to the extended skilled nursing level of care. As a distinct part SNF of our Level I Trauma Center, Community Regional, you will be integral to the continuity of care across our health system.

Responsibilities:
As the Clinical Nurse Manager (also known as the Director of Nursing or "DON"), at our Subacute and Transitional Care Center, you will join a dynamic and growing team! Patients in our facility range in acuity, including hospice, rehabilitation, and general nursing care in a long-term care setting. With your excellent leadership and clinical experience, you will continue fostering advancement and growth, ensuring high-quality, evidence-based practice.

With the support of our around the clock House Supervisors, Quality and MDS nurses, and a staffing ratio of 4.0 or higher, you will have the dynamic support to oversee operations, assess the care of patients, staffing, and coordination of the quality improvement process. You will have the unique opportunity to work with a large scope of patients and team members, while also collaborating to provide an optimum level of care and service.
Qualifications:
Education & Experience

• Graduate from an accredited School of Nursing or successful completion of sufficient units in nursing to qualify for state licensure required
• 3 years of relevant experience in the healthcare industry or a healthcare setting required
• Demonstrated expertise in the service area and leadership ability required
• Previous leadership experience in the healthcare industry or a healthcare setting required
  Licenses and Certifications
  
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • RN - Current State of California Registered Nurse license required
