JOB SUMMARY
The Case Manager I reports to the Program Director and is
responsible for providing case management and assisting program
guests in their activities and in their progress toward
daily/weekly/long-term goals as needed.
ESSENTIAL JOB RESPONSIBILITIES
- Complete resident service plan, assessment, HMIS data
collection, housing plan and help the resident to achieve
identified goals.
- Provide residents with instruction, as related to specific
housing needs.
- Assure that the residents understand and follow program rules
and procedures, by explaining rules and procedures to them and
providing each resident with a copy of the rules and
procedures.
- Complete documentation as need to include specialized incident
reports, crisis intervention, reported medication changes, and
changes in behaviors.
- Maintain relevant relationships with all referral
sources.
- Work along with other staff, outside agencies and assigned
navigators to provide assistance with housing referrals, filling
out rental documents and other pertinent information for
housing.
- Serve as a resource person with regard to decision-making
challenges that may arise.
- Prepare and maintain all files in a confidential
manner.
- Provide information, referrals, follow-up for appropriate
services and resources. Linkage with community resources focusing
on housing.
- Attend navigator case conferencing meetings.
- Fill in for absent staff as requested.
- Observe and address correction of assigned household duties
including all safety issues.
- Become familiar with the "California Tenant Laws" and how they
relate to the HUD Permanent Housing Program.
- Will drive on Agency business as needed.
- Think and act quickly and efficiently in emergencies.
- Physical presence in the office is required.
- Comply with all Turning Point Safety policies and procedures,
including but not limited to: workplace safety, reporting work
related injuries, Infection Control and preventing potential safety
risks for staff, clients and others.
- Punctual and regular attendance is an essential responsibility
of each employee at Turning Point. Employees are expected to report
to work as scheduled, on time and prepared to start working.
Employees also are expected to remain at work for their entire work
schedule. Late arrival, early departure or other absences from
scheduled hours are disruptive and must be avoided.

ESSENTIAL JOB REQUIREMENTS
- Possess a Bachelor's degree in Counseling, Psychology,
Sociology, or related field OR; an Associates of Arts degree and a
minimum of two years' experience in a residential setting or
shelter program OR; a high school diploma/GED and a minimum of two
(2) years' experience in a residential setting or shelter
program.
- Ability to work effectively with program residents, who are
diagnosed with chronic mental illness.
- Ability to obtain certified training in emergency first aid
procedures and CPR.
- Ability to communicate effectively in written and spoken
English.
- Ability to operate Homeless Management Information System
(HMIS).
- Intermediate computer skills in Microsoft Word, Excel, Outlook
and PowerPoint, Office 365, Teams, Zoom and other
communication/meeting platforms.
- Possess a valid driver's license and have access to a
dependable means of transportation that is properly insured and
operated in accordance with all laws. Agency insurer authorization
is required.
- Ability to pass a criminal background check.
- Ability to pass a pre-employment drug screen and TB test.
