In this role you will work collaboratively with the Case Management staff and other health care team members to coordinate and execute discharge planning services for patients through the healthcare continuum. You will help assist in the execution of a plan of care to facilitate a safe discharge in a timely manner and serves as a liaison on community facilities and resources. With a patient ratio of 1:45 your day is anything but average.


1. Screens patients on a daily basis for appropriate discharge planning interventions and assesses patient discharge planning needs.

2. Collaborates with the Case Management team, the patient and family, the physician, hospital staff, and other care providers to determine post-hospital care needs.

3. Follows the patient through the discharge process to assure all aspects of the discharge plan have been met.

4. Maintains an accurate and current source of appropriate community based facilities and services where the patient can be transferred or referred.

5. Performs appropriate referrals to community agencies and resources.

6. Ensures all elements critical to the discharge plan have been communicated to patient/caregiver and the multidisciplinary team; and documented in the electronic medical record.

7. Contacts receiving facility (SNF, subacute, acute; board and care; etc.) to verify acceptance and confirm transfer date/time.

8. Assists in the coordination of transportation set up and notifies Point of Service RN, RN Case Manager and/or social worker of final plan.

9. Delivers the Medicare Important Message Letters, SB 1076 Letters, and Medicare Observation Outpatient Notification (MOON) letters.

10. Makes appropriate referrals to admitting department for assistance with identifying financial benefit services.

11. Maintains daily written log to track daily SNF placement efforts, per MediCal requirements and Revenue Cycle policies/procedures.

12. Documents discharge planning arrangement to meet CMC and Case Management documentation standards as indicated in CM policy and protocols.

13. Performs miscellaneous job related duties as assigned.



* One of the following is required

o Bachelor's Degree in Social Work or related field

o Completion of an accredited Vocational Nursing School program


* 1 year of experience in Case Management with prior assessment and referral experience required
* Experience in discharge planning in an acute care hospital or skilled nursing facility preferred

Licenses and Certifications

* BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
* LVN - Current State of California Licensed Vocational Nurse License for incumbents without a qualifying Bachelor's Degree required
