Overview: Community Regional Medical Center is the flagship hospital of Community Medical Centers a private, not-for-profit healthcare system based in Fresno, California, and the largest healthcare provider and private employer in central California. Community Regional Medical Center is seeking nurses like you for challenging 8 hour shifts! In this role you will work in close partnership with Social Workers, Discharge Navigators and the Interdisciplinary clinical team. RN Case Managers gather information from a wide variety of resources to create, implement and deliver safe discharge plans for our patient population. They use evidence based software for review and streamlined technology for discharge patients. Qualifications: Experience and education minimum requirements:
  • Bachelor's degree in Nursing
  • One year acute care hospital experience.
    • Three years of acute care hospital experience may be substituted in lieu of education/experience.
    • Current California Registered Nurse license.
    • Current BLS certification. PREFERRED:
      • Master s degree in Nursing
      • One to two years acute Case Management experience.
