Overview:Community Regional Medical Center is the flagship hospital of Community Medical Centers a private, not-for-profit healthcare system based in Fresno, California, and the largest healthcare provider and private employer in central California. Future plans include building a women and children s facility on campus and expanding with 65 additional beds. In this role you will work in close partnership with Social Workers, Discharge Navigators and the Interdisciplinary clinical team. RN Case Managers gather information from a wide variety of resources to create, implement and deliver safe discharge plans for our patient population. They use evidence based software for review and streamlined technology for discharge patients.Qualifications:Experience and education minimum requirements:
  • Bachelor's degree in Nursing
  • One year acute care hospital experience.
    • Three years of acute care hospital experience may be substituted in lieu of education/experience.
    • Current California Registered Nurse license.
    • Current BLS certification.PREFERRED:
      • Master s degree in Nursing
      • One to two years acute Case Management experience.
