Quality.HGS AxisPoint Health offers top compensation, great benefits and collaborative, welcoming work place!"Driven by Innovation, Powered by People."Responsibilities & Qualifications:Position DescriptionThe Case Management, Registered Nurse (RN) provides case management and disease management nursing services and care coordination to high risk Medicaid patients in a geographical region. The RN provides education, counseling and on-demand information to patients who have enrolled in a condition specific program. The RN will provide program education and coordinate care needs with the patient's Primary Care Provider. Services are performed telephonically from a home office (50% of the time) and at field locations (50% of the time). The location is in the Fresno, CA area.Location / Work Environment

• Independent, home office based environment. Must have access to a high-speed internet connection and be able to comply with work-at-home agent requirements.Position Qualifications
  • RN with active, unrestricted nursing license within state position is located
  • RN responsible and accountable for safe clinical practice, knowledgeable of customers, and regulatory expectations in the clinical area
  • A minimum of three years of clinical experience
  • Two plus years' experience as a Certified Case Management RN
  • Entry level clinical telephonic and field experience may include basic knowledge of nurse call center procedures and/or company products
  • Excellent communication and telephone skills
  • Full competency in Microsoft Office Suite
  • Strong computer technology/navigation skills
  • For patient and provider facing staff, must have:
    • Current CPR certification
    • Current PPD
    • Current valid driver's license and reliable transportation with required level of automobile liability insurance coverage
    • Bilingual in Spanish a plus
    • Must have a Case Management CertificationEducation and Professional Certifications
      • Associate Degree in Nursing
