Job DescriptionPOSITION SUMMARY:Responsible for supervision and training of individuals with developmental disabilities. Works closely with Case Manager & Program Manager to assist individuals in the development of their skills to their full potential, and foster their independence in a positive and caring manner. Completes required documentation.

DUTIES AND RESPONSIBILITIES:

• Use the established methodology for achieving client goals and track progress.
• Provides care, supervision, instruction and/or job coaching for the individuals we serve, services might be provided in person, remotely or in the community.
• Participates in weekly online videos.
• Development of activities, curriculum and schedules for programming in person or remotely.
• Assists the Case Manager in implementation of the Individual Service Plans (ISP's)/Individual Habilitation Component (IHC) and provides documentation as directed by the Program Manager.
• Assists with personal hygiene, toileting, feeding, and ambulation needs as needed.
• Accurately and legibly evaluates and documents the individuals' progress through daily charting and log notes in client electronic file. JOB REQUIREMENTS:
  • High school diploma or general education degree (GED). Prefer three to six months of related experience working with developmentally disabled individuals or other special populations.
  • Must possess valid California Drivers License. Requires ability to pass background check, physical, Tb and drug test.
  • Must be able to push and pull up to 50 pounds.
